Services
CFSS CHOICE:
Feel better in the comfort of your own home. We specialize in care and daily living assistance to an array of individuals. Whether you need daily or weekly assistance due to aging, illness, recovery, or rehabilitation, our care givers will provide an individualized service that you can trust.
Home Health Aides/ CFSS Services:
A CFSS (personal Care Assistant) also known as a home health aide is a individual who is trained to provide services to a person(s) in their home, who needs help with their (ADLS); activities of daily living to help them maintain their personal health; which allows them to maintain their independence and remain in their own home.
Homemaker/Companionship Services
Companion/Homemaker provides companionship to those individuals requiring socialization and/or minimum guidance to assure a protected environment and performs home management services within the clients home. Homemakers are generally responsible for keeping the client’s home clean, and keep things running smoothly. You decide what cleaning jobs need to be done.
Waivered Services
Waivered services foster the developmental skills for individuals with special needs/disability in both home and community. Services are designed to meet individual and family needs for clients using DD, Cadi, CAC and BI waivers; referrals are made by social workers in Hennepin and Anoka Counties. Services are funded through the individuals waiver and/or consumer directed community supports. (Recipients must be eligible for MA)
Respite Services
Respite services are short-term or temporary care provided to you or your loved one in their home or a designated place of your choice, where you will be cared for by a different caregiver. Respite care is provided in the case of an emergency or crisis, or it can also be arranged ahead of time, depending on the plans of the caregiver. Respite care is flexible and can be used for just a few hours or for several days or weeks at a time.
